EQRS Clinical Data Submission Deadlines
for CY 2025 Data

The Centers for Medicare & Medicaid Services (CMS) End Stage Renal Disease (ESRD) Quality Reporting System (EQRS) data submission deadlines for the remainder of calendar year (CY) 2025 are listed below. These deadlines apply to all collection types (Hemodialysis and Peritoneal Dialysis) and all submission methods. The remaining CY 2025 EQRS data submission deadlines are listed below for the following:
- EQRS Clinical Data
- Clinical Depression Screening and Follow-Up Assessments
- In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Attestation
EQRS Clinical Data opens for submission on the first day of the month and typically closes approximately two months after the end of the reporting month. EQRS deadlines occur on the last day of the month. However, if the last day of the month falls on a Friday, Saturday, Sunday, or federal holiday, the deadline will occur on the next federal business day.
